I have taken my time to go through the steps but this is the message I receive.

Hi,

Thanks for coming back to us on this. I notice from your screenshot that you are downloading a version 4 report template, but you're using ClinicOffice version 5. Please download the correct report template for your version of ClinicOffice.

Please go back to the instructions here : viewtopic.php?f=19&t=249

...and download the ClinicOffice v5 templates that you require (not the v4 templates).
